Water-saving in this catalogue is almost always built in: a restriction moulded into the head that cannot be removed. Here it is a loose seal in the box.
On strong mains pressure, fit it and use less water. On a weak or gravity-fed supply, leave it out and have a usable shower. That decision belongs to whoever lives there, and this is the only set in the hub that hands it over.

Because it is reversible. Most water-saving heads restrict flow permanently and cannot be opened up. Here the restriction is a washer supplied loose: on strong mains you fit it and save water, on a weak supply you leave it out and still have a shower.
